Gynecomastia also referred to as male boobs is an enlarged breast-associated condition that is known to affect a large percentage of the male population (especially adolescent boys and older men) because of a hormonal imbalance i.e. rise in estrogen levels which stimulates swelling up of breast tissues. This condition is characterized by the presence of excess glandular tissue, excess fat, and skin that mimics the appearance of female breasts. In this condition, men experience breast tenderness as well as changes in color or position of their nipples. ‘Although gynecomastia does not have an impact on an individual’s health it can be a setback for people in terms of confidence,’ shares Dr. Mrinalini Sharma, Gynecomastia Surgeon in Delhi at Aestiva Clinic. The condition causes emotional distress in affected individuals and causes them to hide their enlarged breasts by wearing a loose t-shirt or wearing tight vests.

Factors that contribute to gynecomastia:

Men can experience gynecomastia due to various reasons such as hereditary issues, daily intake of certain medications like for heart disease, medical conditions like hyperthyroidism, hypogonadism, and kidney/ liver disease, increased estrogen levels, alcohol consumption, anabolic steroids, chemotherapy, narcotics, amphetamines, low testosterone levels, or other health complications.

Gynecomastia needs to be addressed soon before it leads to extreme discomfort like rubbing or chafing of the skin. This is possible by male breast reduction treatment called gynecomastia surgery. People who are mentally and physically fit and have a relatively healthy body weight are eligible to undergo gynecomastia surgery. At Aestiva Clinic Dr. Mrinalini Sharma, plastic surgeon in Delhi performs this life-changing surgery and helps one to achieve the desired masculine chest by correcting the enlarged breasts in males.

Stages of gynecomastia:

When gynecomastia first starts developing, breast pain could be startling. The first sign of gynecomastia is usually the appearance of a lump of fatty tissue which may feel tender to touch as the swelling continues but the sensitivity usually subsides. There are grading systems designed to classify the various degrees of gynecomastia condition, including:

•          Grade I – Minor enlargement without the excess breast tissue (less than 250 grams of breast tissue)

•          Grade II (a) – Moderate enlargement without excess skin (250-500 grams of breast tissue).

•          Grade II (b) - Moderate enlargement with minor excess skin (250-500 gram of breast tissue).

•          Grade III – Severe enlargement (more than 500 grams of breast tissue) with grade I sagging.

•          Grade IV – Severe enlargement with grade II or grade III sagging.

How is gynecomastia surgery performed?

Gynecomastia surgery is carried out to restore a normal masculine chest and to correct deformities of the breast, nipple, or areola. The common procedures involve liposuction, surgical excision, or a combination of the two. They are performed with either local anesthesia and sedation or general anesthesia.

1. Liposuction – People with mild to moderate gynecomastia, involving breast enlargement with minimal gland hypertrophy and no excess skin are good candidates for undergoing liposuction. Power-assisted liposuction (PAL) or ultrasound-assisted liposuction (UAL) are two approaches for the effective removal of fibrous tissue from breasts. During the liposuction technique, the surgeon creates several small incisions through which a cannula (a thin hollow tube) is inserted. The cannula helps in loosening stubborn fat deposits from the breasts as it is moved back and forth in a controlled motion. The excess fat is later removed from the body by using vacuum suction. This method compromises less on the supply of blood, areola slough, and nipple distortion.

2. Tissue Excision – This procedure is carried out when there is a requirement of reduction of areola or repositioning of nipple to a more natural male contour. It involves the removal of glandular breast tissue or excess skin. Individuals with a small amount of fibrous glandular tissue under the nipple can be treated with direct excision by making a small incision around the areola (peri-areolar incision). The pattern of incisions may vary depending on the extent of surgery needed and specific conditions. Additionally, this method is apt for people who have sag, stretched skin around the breasts. The cost of gynecomastia surgery in Delhi depends on several factors like the amount of fat removed, excess skin removed, and the type of surgical procedure performed. The gynecomastia surgery cost in Delhi in Aestiva Clinic is true value as Dr. Mrinalini Sharma focuses on patient safety and safe surgical outcomes.

Pre-operative instructions:

•          Avoid intake of any food item or beverages for at least six hours before the procedure.

•          Avoid taking anti-inflammatory medications for two weeks before the procedure.

•          Stop smoking for four weeks beforehand as it decreases blood flow to the skin and may delay the healing process.

After-care instructions:

•          The patient must finish the course of prescribed antibiotics to prevent any post-operative infections.

•          Avoid lifting anything heavy for three to four weeks and also do not indulge in contact sports, sexual activity, or vigorous physical activities for a few couple of weeks. A sedentary daily routine can be performed after few days as the surgeon recommends.

•          Keep the body hydrated.

•          At least for the first 3 days, sleep or rest on the back at a 45-degree angle.

•          Go for follow-up visits to the doctor who has performed surgery for removal of stitches, skin tapes, or surgical dressings. Take care of the bandages or binder around the chest until removed.

•          Do not smoke for a few weeks and avoid direct sun exposure over incisions for at least 6 weeks.
